Home and Community Based Services (HCBS)

  • Respite

    Respite services will provide short-term relief for the primary caregiver. These services can be provided any time of the day or evening that the caregiver requests.

  • Habilitation

    Habilitation services are for individuals that need assistance with working on outcomes that are described in a Person Centered Service Plan. The certified habilitation worker will come and help the individual meet their outcomes in the home or community.

  • Attendant Care

    Attendant care is to help an individual with their personal hygiene, assist in dressing, and helping with meals. Attendant care can be received in the home or community.

Want to become an HCBS Provider?

  • Level I Fingerprint Clearance Card

  • CPR Infant, Child, Adult AED Training

  • First Aid Training

  • Article 9 Training

  • Prevention & Support Training*

  • Direct Care Worker Training*

  • 18+ years of age

  • High School Diploma or GED

Requirements

  • Provide care consistent with the standards established by the family and/or the contracting agencies

  • Document unusual incidents promptly and thoroughly

  • Must maintain client confidentiality at all times

  • Must abide by all agency policies and procedures

  • Maintain all training requirements

  • Maintain clear DPS Fingerprint background check

  • Follow through with guiding the child/adult through established tasks, measuring their activities, and documenting their progress towards meeting their goals
